STEP 2: Liability Release Form
Please read carefully and make sure you agree with all the terms and conditions mentioned below. You must fill out the form below.
We realize that work and personal commitments may change, and the date originally booked may no longer be suitable. It is vital that our office is notified at the earliest possible time, if you are unable to attend the course.
Please note that all cancellations and transfers must be made in writing and emailed to karen@laughtermaryland.com We will confirm receipt of your email within 48 hours. If you do not hear back from us within 48 hours of your cancellation or transfer request, you must call us at (301) 602 1760.
• If notice of a transfer or cancellation is given up to fourteen days prior to course commencement, there is an administration fee of US $50.00 per person.
• If cancellation is made between seven and fourteen days prior to course commencement, a fee of 50% of the course value is payable.
• If a cancellation is made between forty-eight hours and seven days prior to course commencement, 100% of the course value is payable.
• If a person does not attend and notice of cancellation or transfer has not been made in writing then the invoiced amount is still payable.
• Transfers, credit notes and gift vouchers are valid only once. If you are unable to attend the course the full amount is still payable.
• Replacement participants are welcomed at no additional cost.
Event Cancellation Policy
• Anything can happen any time to anybody, including us. Laughter Maryland Inc, reserves the right to cancel any program at any time.
• If we cancel a program, you will receive a full refund. We recommend you contact your travel agent to purchase travel insurance in the event that you need to cancel or change your travel plans.
Workshop Fees
• A non-refundable deposit of $50 will secure attendance to the requested workshop.
• All fees must be paid in full at least 5 days before the start of the event.
Insurance
• Each Individual is responsible for his or her own insurance needs (be it accidental and or medical). Laughter Maryland does not take responsibility for the misuse of the training facility equipment and surrounding property resulting in the injury to any person within the participating group or organization while attending one of our events.
Disclaimer
The Healing Power of Laughter and Laughter Tea Events is taught by Laughter Maryland. These workshops are a fun & healthy exercise regime with beneficial effects for the body, mind & emotions. Like any form of aerobic exercise, it should not be practiced by people with medical problems without first taking advice from their medical practitioner.
Laughter Therapy should not be considered as a substitute for proper medical consultation for physical, mental and psychological illnesses.
It may not be suitable for everyone. Laughing involves some physical strain and a rise in intra-abdominal pressure. It is contraindicated for people suffering from.
• Advanced (bleeding) piles
• Any kind of hernia
• Any persistent cough
• Anything with acute symptoms
• Epilepsy
• Heart disease |
• High blood pressure
• Incontinence of urine
• Major psychiatric disorders
• Severe backache
• Or having undergone surgery within the last 3 months |
This list is for guidance only and is not meant to be exhaustive. If in doubt, first consult a trained medical professional for guidance. Anyone already undergoing physician-prescribed therapy should seek the advice of their doctor before reducing or stopping such treatment.
Please use your common sense. If during a session you feel sudden or new pain please leave the session immediately, or advise the workshop teacher if you require assistance.
What You Can’t Bring
• Illegal / hallucinatory drugs of any kind, alcoholic anything and pets are not allowed in any of our trainings.
• Please keep your cell phone switched off or on vibrate mode during all work sessions.
